Family Day Charity Scramble Entry Procedure



Entry Procedure

 
A) Application to enter the OSA Family Day Charity Scramble must be made on the Entry Form linked below.

B) The entry form must be received by no later than Monday, February 8, 2010

C) The Entry Fee is $250.   Cheque or money order must be made payable to The Ontario Soccer Association and must be received by the Entry Deadline. If a team
has submitted an online entry before the deadline, but payment has not been recieved by the OSA Provincial Office by the Entry Deadline that team will not be guaranteed entry into the Competition.

D) Each team entering the Tournament from outside Toronto S.A (for U-10 at Lamport) or York Region S.A. (for U-12 at Soccer Centre) must obtain Permission to Travel from its District Association by the entry deadline and submit it online, by fax or e-mail at the time its registration form is submitted in order to be considered registered. Failure to present a Travel Permit will result in forfeiture of all scheduled games without refund.

Please note that because of limited spaces, entries will be accepted on a first come, first served basis.
